This extension and website functionality are intended solely for use by the administrator of our medical practice. However, since patients can interact with the website to view their upcoming appointments, we want to clearly explain how your information is handled.

 

What information is collected?

For the purpose of displaying your upcoming appointment, the following information may be used:

• Your name

• Your date of birth

• The last four digits of your Social Security Number

• Your appointment date and time

This information is not collected by Google and is not shared with third parties. It is stored only in a secure database we maintain.

 

How is your data used?

• The Chrome extension is used only by our practice administrator to manage appointments.

• When your appointment is created, your name, date of birth, and last 4 of SSN are added to a secure, private database hosted on our practice website (Wix).

• This allows you to visit our site and securely look up your next appointment by entering the same information.

• No other use of your data occurs.

 

How is your data secured?

• All communication between our extension and website is encrypted over HTTPS.

• The backend is protected using CORS restrictions, allowing only authorized connections from within our extension.

• We do not use cookies, tracking, advertising, or analytics tools.

 

Can I opt out or delete my information?

Absolutely. Participation is voluntary. If you prefer not to use your Social Security number or wish to have your record removed from our appointment lookup system, just contact our office — we’ll delete your information promptly.
